You can provide your feedback in the following ways:

Complete a feedback form

We have produced a feedback questionnaire for you to fill in and submit. Paper copies are available to pick up from our face-to-face consultation events and our consultation information points. You can also download and print a copy of the feedback form from our website and return to us for free by post; alternatively, please get in touch and we will post one to you.

You can also submit your comments digitally using our online feedback form.
