We would be grateful if you could complete the form as fully as possible and return it. Once we have received your application we will send you a Quotation for the work, together with an Acceptance and Payment Form for you to sign and return.

If the gas service pipe you require is for a newly built property, please supply a scale plan of the building within the plot, together with a location plan of the plot and a clear indication of the plot or property number.

Important information

We recommend that you read the enclosed information thoroughly, including the following important key points:

• The quotation we will provide you with will be for a new gas service pipe only. Under current industry arrangements, you will need to contact a Gas Supplier to arrange for the installation of a meter and a gas supply contract. Your gas supply will not become operational until this is completed.
• If you require a Built-In Cavity Meter Box, you will need to purchase one yourself and ensure that it is correctly installed before we start work on site. This type of meter box can be obtained from a variety of outlets, such as builders merchants, although we can provide you with further information on potential stockists if required.
• On receipt of your signed Acceptance Form and cleared payment, we will contact you within four weeks of receipt to arrange a convenient date to commence the works on site.
• Please remember that we will need to access your property to complete the work.
• Before signing the Acceptance Form that we will send you with your Quotation, please carefully read the terms and conditions printed on the reverse of the enclosed Application Form.
