You will need to provide the UIP with some key information to assist them with your request, so before completing the form please ensure you have the following information.
- Customer name – the name of the person submitting the request (not your clients name or site name, etc).
- Postcode – this is the postcode of the location where the works are to be undertaken. If the postcode is not known then please enter n/a in the postcode field and provide as much information as possible in the site address details.
- E-mail address – you will need to provide a valid e-mail address that the UIP can use to contact you to process your request.
- Type of work – enter the category of work you require from the above list.
Please use the additional comments field to add anything that might assist the UIP with your request (important site information, alternative contact details, etc).
